WeCare TLC Strategy- establish PCMH (Primary Care Medical Home) clinics throughout the metropolitan Quincy/Hannibal/Mt Sterling/Jacksonville area that are available to the Tri-State Health Care Coalition employers’ employees and dependents. Not only are robust primary care services offered, but also occupational health, chronic care management and convenient care.

Every clinic would be available to all eligible members based on their individual convenience and preferences. All services performed within the clinic would be at no cost to the patients. No insurance claims would be created and will lower your claims experience. The services within the clinic would sit in “front” of the medical plan and as such would not require any health plan changes, although some employers may elect to make changes in their plans as a results of this service.

 WeCare TLC, assumes responsibility for locating, negotiating and equipping necessary clinic locations.
